Overview
Tell the GNWT you are moving away from the NWT so your health care coverage can end at the right time. This page explains how moving within Canada or outside Canada affects your NWT health care coverage and how to report your move.
Tell us when you are moving away
Use this process if you are:
- Moving to another province or territory in Canada
- Moving outside Canada for a permanent relocation
You must notify the Health Services Administration Office when you move away from the NWT.
If you are only away for a limited time and plan to return to the NWT, use Keep your coverage during a temporary absence instead.
Understand how your coverage changes when you move
When you move away from the NWT:
- There is a 3 month transition period while you switch to coverage in your new province or territory
- Coverage outside Canada is limited.
Your new province or territory is responsible for your coverage after you become a resident there.
Check with your new health plan to confirm when their coverage begins.
What you need before you report your move
You may need:
- A completed Notification of Change Form
- Your new address and contact information
- The date you are leaving the NWT
- Any documents requested on the form that confirm your move

How to report your move
- Download the Notification of Change Form.
- Gather any information or documents needed to confirm your move.
- Complete every section of the form.
- Submit your form by email, mail, or fax to the Health Services Administration Office.
- Keep a copy of your form and documents for your records.
Understand what happens after you report your move
The Health Services Administration Office reviews your form and updates your NWT health care coverage based on your move date and portability rules.
You may be contacted if more information is needed.
Your health care coverage will end once you are no longer an NWT resident under the NWT Health Care Plan Policy.
